ဒီ patient ရဲ ့ breast lump biopsy result ရၿပီလား။
မရေသးပါဘူး ဆရာႀကီး။
တကယ္လို ့ cancer ျဖစ္မယ္ဆိုရင္ ဘာ အမ်ိဳးအစား ျဖစ္ႏိုင္မယ္ထင္လဲ။
Ductal carcinoma ျဖစ္ႏိုင္ေလာက္ပါတယ္ ဆရာႀကီး။
ဘာလို ့လဲ။
Commonest type မိုလို ့ပါ။
ေအးေပါ့ ဒါကေတာ့ common thing is common ေပါ့ေလ။ may be Ductal carcinoma in situ or may be invasive Ductal carcinoma ေပါ့။ တကယ္လို ့ biopsy result က lobular carcinoma လို ့မ်ား အေျဖထြက္ခဲ ့ရင္ ဘာစဥ္းစားမလဲ။

မေျဖတတ္ဘူးဆိုရင္ ေနာက္တစ္မ်ိဳး ေျပာင္းေမးမယ္။
တကယ္လို ့ biopsy result က lobular carcinoma လို ့မ်ား အေျဖထြက္ခဲ ့ရင္ mammogram and ultrasound result အေပၚ ဘယ္လိုသံုးသပ္မလဲ။
ဟုတ္ကဲ ့ပါ ဆရာႀကီး..Lobular carcinoma မွာ abnormal lesion ဆိုလိုတာက breast lump က diffuse and less obvious ျဖစ္တတ္တဲ ့ သေဘာရွိပါတယ္။ အဲေတာ့ mammogram and ultrasound တို ့result မွာ disease's condition may be understaged ျဖစ္သြားႏိုင္ပါတယ္ ဆရာႀကီး။
အေရးႀကီးလား။ ဘာလို ့လဲ။
ႀကီးပါတယ္ ဆရာႀကီး။ inadequate Tx ျဖစ္ေကာင္းျဖစ္သြားႏိုင္ပါတယ္။
အဲေတာ့ ဘာလုပ္မလဲ။
MRI ႐ိုက္ပါ့မယ့္ ဆရာႀကီး။
ေအးဟုတ္ၿပီး ေျပာခ်င္တာက challenge for radiological detection of lobular carcinoma of breast ဆိုတာရွိတယ္။ ဘာလို ့လည္း ဆိုေတာ့ less cohesive pattern of lobular carcinoma ေၾကာင့္ mammogram တို ့ Ultrasound တို ့မွာ lower ability to recognize or underestimate ျဖစ္ႏိုင္တယ္။ even may be mammographically occult ျဖစ္ႏိုင္တယ္.
ဆိုေတာ့ lobular carcinoma နဲ ့ patient ၊ အထူးသျဖင့္ breast conserving surgery လုပ္ခ်င္တယ္ဆိုရင္ MRI breast က ႐ိုက္ကို ႐ိုက္ရမယ္။ ultrasound, mammogram တို ့မွာ may be understaged ျဖစ္တတ္တယ္။ inadequate treatment ျဖစ္သြားမွာ သတိထားရမယ္။ ၿပီးေတာ့ lobular carcinoma ေတြက ပိုၿပီး Multifocal or multicentericity ျဖစ္တတ္တယ္။ contralateral breast ကို metastasis ျဖစ္တတ္တယ္။
ေအးပါ biopsy result ၾကည့္တာေပါ့။
ဆရာႀကီးေတြက စာသင္ေပးခ်င္ရင္ အဲလိုပါ။ biopsy result ေတာင္ မထြက္ေသးဘူး။ တကယ္လို ့ lobular carcinoma breast လို ့ အေျဖထြက္ခဲ ့ရင္ radiological challenge ဆိုတဲ ့ အခ်က္ေလးက အၿမဲထည့္စဥ္းစားရပါ့မယ္ လို ့သင္ေပးခဲ ့တာပါ။
မွတ္ခ်က္ ။ ။ ဒီ post သည္ Mx of Ca breast ကို ေျပာျခင္း မဟုတ္ပါဘူး။ Mx ေတြထဲက စဥ္းစားရမယ့္ အခ်က္မ်ားစြာထဲက တစ္ခု (ဆရာႀကီး တစ္ေယာက္သင္ေပးထား) ကို ေျပာျခင္းသာျဖစ္ပါတယ္။
